Attend doctor appointments and facilitate communication between doctor, client, and family. Determine types of services – including home health and hospice and in home care – that are right for the client and assist in engaging and monitoring those services.
Estimate costs of current and future care needs whether at home, assisted living or skilled nursing.
Refer to and assist with accessing additional resources.
Help clients evaluate and select appropriate level of housing or residential options.
Advocate for clients and their families by promoting the client’s wishes with health care and other providers, ensuring that client’s needs are being adequately addressed.
Referral to elder law attorneys, estate planners, and fiduciary services. Assist with advance planning documents.
Helping clients navigate through emergency departments and hospitalizations, rehabilitation stays, and ensuring that adequate care is provided to the client.
